Current vent is Gorton #1 and I would like to upgrade. The vent is a bit tilted. Not sure if it affects the venting but I have no issues.

Big Mouth should fit. It is a 2 part unit. You screw the bottom half of the union into the pipe opening and then you can hold the top part (the actual venting portion) in place and tighten the union onto the part that you screwed into the pipe. The entire unit does not need to turn. Flat side towards the joist and venting side out.

why not use a Barnes and Jones vari vent? I use them and love them.

For more head room replace the nipple and elbow with a street 90?—NBC

If the nipple would come or not, I would add a 90 with a long nipple and an another 90 with vertical riser if possible.
IMO the more length and 90's you have the less chance of water hammer, debris and actual water hitting your new vent. You want the horizontal nipple sloped to drain back into the main pipe.
