My apartment has forced hot air and these medieval looking registers. Has any one replaced this with a regular register? What am I dealing with here?

Looks like that thing has a thermostatic damper regulator built into it. Not sure a standard register would work there.All Steamed Up, Inc.

Agree with @Steamhead. It looks like the outer box was part of the rough in so it's probably nailed to the studs.
The grill/damper assembly was installed after sheet rock (plaster?) and paint.
First, measure the outer dimensions and see if that size is offered in a VM grill. They have manual dampers. If not, a stamp grill will work, but no damper as it is a surface mount.
Remove the grill/damper assembly. Cut the exposed box to within 1" of the wall. Cut "V" notches at the corners. Use flange pliers and bend the exposed 1" 180 degrees back into the box. This will allow for a recessed VM grill if feasible.0
