Boiler Not Firing Consistently When Thermostat Calls

We have a a Utica PEG187BDE that runs the single-pipe steam system in our house. Last year our heating maintenance company replaced the Honeywell gas regulator and our old thermostat with a Honeywell Focus Pro TH6000 series.

This year we have had increasing problems with the boiler not firing when the thermostat calls for heat. The thermostat calls down to the boiler, I can hear the relay on the side of the unit beeping, the pilot lights, and than after several seconds the regulator shuts completely. Usually I have to cycle the electrical power switch o the boiler 3-5 times to force this cycle of events before finally the regulator opens and ignites the burners.

We have noticed that if the boiler is still hot/warm during a multi-cycle heat-up the boiler igniting without any problems.

Any ideas as to what is going on?

    1. Verify ignition module sequence and that ignition module is functioning.
    2. Check for a good ground from burner to ignition module.
    3. Check for a clean pilot flame sensor rod
