Freeze kits

Anyone ever have a freeze kit black head explode on them...it was a 11/4 black iron pipe I was freezing a while back, it was not pretty...I thought of this because I was cleaning up and found the general freeze kit and recalled why it was missing the 11/4 head , one of two.
