Anyone have experiences with the board going bad and the transformer going bad at the same time without blowing either fuse on the board? Any Idea what could cause this?

updated board with the green dot on it?0 -
Not sure about the green dot, but it is version 5 and was replaced with version 5.0

Not that there were many comments, but I thought I should close this out.
As it turns out it seems that the issue was with the unrelated work the electricians were doing in installing a backup generator. After talking to another electrician and an electrical engineer, it seems probable that that at some point when they were transferring the circuit they inadvertently sent 220 to the boiler circuit. we found out later that there were three other transformers that were also shot (2 75 va's running the zone valves and the transformer in the taco SR502). again no fuses blown.1
