Re-piping improper install of pipe to one radiator...

We have 1-pipe steam heat. When we renovated our bathroom a year ago, the radiator in the bathroom was relocated to a different spot. Because of improper pitch, there is a lot of water hammer. The bathroom where this radiator is installed is on the 2nd floor. Recently, we noticed a leak in the first floor ceiling and immediately realized this piping was screwed up and was not proper.
We want to fix this ourselves because it seems like a job we can handle. We have exposed the 1st floor ceiling right below the leak and want to re-pipe in such a way to maintain proper pitch going upwards into the radiator.
The piping from the main (basement) is properly pitched upward, From the basement, pipe that travels vertically through the first floor (just straight upward, no angles) and makes its way into the ceiling first floor ceiling and then inwards, parallel to the ceiling (between the 1st level's ceiling and the 2nd level's floor). We have about 9-12 inches of vertical space between the ceiling and the floor, where we need to properly pitch this pipe.
The horizontal pipe that will travel between floors is about 44-45 inches long. How much of a pitch do we need here? at a minimum?

Repiping
Where did the water in the leak come from?
If from a vent, what make of vent is it?
What is the diameter of the pipe?
Generally speaking an inch a foot of rise to lateral distance should be adequate-sometimes less.--NBC0 -
The leak came from the pipe that feeds to the radiator
it seems between the ceiling and the floor, the pipe fed to another pipe using an elbow and then the 2nd pipe fed to the radiator. The elbow was where the leak occurred. We are fixing it by install a single pipe to replace the elbow and two pipes. The hammering happened because there was no slope upwards. Infact the slope was downwards.
This is not coming from a vent.
The pipe diameter is 1 inch.0 -
You can get by with less
Assuming it's a small radiator you could get by with an inch or two for the ~4 ft horizontal run, it's not optimal but it should work.
