GB142 UBA, anyone have one go bust?

I have heard of two situations where a technician (not us) did their basic annual cleaning for their customer, and upon starting up the system, zippo. Dead UBA.
I just drafted up a basic letter and sent in to all of our GB142 customers, indicating in several polite sentences that could happen, and what the cost would be to replace it if it did indeed happen.
Has anyone else heard of this? if yes, how on earth did that go with the home owner?
