Can anyone tell me what model boiler this is?

I have a Weil-McLain CGt boiler. I'm looking for the specific model of this boiler but I don't know what it is. I need the model so I can buy replacement parts for the parts in the last two pictures.

http://s231.beta.photobucket.com/user/whatthephil/media/IMG_20130103_174653-1.jpg.html shows a boiler which has suffered from many years of neglect.
If http://s231.beta.photobucket.com/user/whatthephil/media/IMG_20130103_174432-1.jpg.html is correct, your pressure relief valve has failed. You need to shut this boiler down now, as it presents a serious safety hazard.
