Condensing tank failure

We had an intersting replacement yesterday of a condensing hot water tank of just 4 years old. The heat exchanger was perforated. So, the combustion chamber was full of water and it even came out of the joints of the fan. Not just that, there was water in the gas line for some 20 feet! We dismantled the gas line to dry it out.
Good thing that our boys checked the line or it would of killed the gas valve in the new tank.

The only thing that we could deduct was that the combustion chamber was partially blocked causing corrosion of the surfaces which led to city pressure entering the combustion chamber. In the photo, you can see that there is still water at the bottom. It is a 4 year old tank.0
