Hot water boiler water overflow

Friend of mine has W-M hot water boiler, about 20 yr old, he never noticed any water from overflow pipe til this yr, and now during warm spell, boiler not running as much, more so. What would be cause? He has baseboard hot water heat, and separate hot water heater. Boiler is gas-fired.

What would be cause?
1.) Pressure reducing valve for filling the boiler failing?
2.) Old style expansion tank water-logged?
3.) Diaphragm style expansion tank failure?
4.) Pressure relief valve defective.
If this has a hot water coil in use (taking chill off input to hot water heater), it could have a pinhole leak.
Whatever the cause, he does want it fixed very soon, before the relief valve fails shut and the boiler becomes a bomb.0
