Laars Endurance ignitor/flame sensor failure

Hi, I recently had a Laars EBP110 Natural Gas Hydronic Boiler w/ Domestic Hot Water installed in my apt to replace an aging and leaking water boiler/heater that originally came with my apt. It worked fine for a couple of days before we lost heat and hot water. LED screen on EBP110 read LO, Locked Out, and it turns out the ignitor/flame sensor failed. We had it replaced by the installer but a week later that failed too. Am I just unlucky to have 2 ignitor/flame sensor assemblies fail within 2 weeks of each other or is the Laars unit somehow causing the ignitor/flamer sensor to fail prematurely. The installer already looked over the unit but didnt find any electrical or water problem. Any help would be greatly appreciated.

Bad ground
Check your ground on your line voltage feeding the boiler. If conduit is used as a ground or you have aluminum wiring, you need to ground the boiler to a solid ground somewhere.0
