steam cervice checklist

good sirs,

 just wondering if any of you plumbers out there have a steam/hot water boiler service checklist that you use when performing a start up service at a customers house?i am compiling one for my techs and i wanted to make sure i didn't leave anything out .also do any of you have maintainance agreements with your customers. how do they work for you and are they profitable are just a pain.
