Problems with plus 100 indirect water heater

I'm a HVAC contractor in Iowa, we are having issues with a plus 100 that was installed by another contractor.We first discovered a collapsed tank on the plus 100 because it was filled improperly. We changed it out last year and the system ran great for a year. Well now it is acting up again. I was wandering if anyone else out there has had issues with this setup?
