cosmogas wall hung

in THE MAIN WALL
customer has cosmogas wall hung (embonex45wn) boiler installed 12-2008. customer states it make a loud screaming type noise when first coming on when running hot & cold (tempered) water i.e washing hands taking a shower. it only last a few seconds. company has been out twice but has yet to hear this noise. when unit was first put into service it made a noise like this but only on first startup boss wrote off as wierd first startup kind of thing.
has anyone seen this before? found the problem? had factory support help fix? i havent heard noise first hand and can not get it to make it. put maybe two hours into cycling unit with no noise, so im drawing a blank on where to start or how to fix. all looks ok other then this noise. gas pressure good water pressure good exhaust and intake clear, it fires and heats (house or how water) just as it was made to do
