the National-U.S. Radiator Co. in the early 1960s or so. You might have a re-badged National or U.S. boiler.
The most likely candidate is the National 4-series. The 12-4 has an input rating of 619 MBH, EDR on steam 2060 (494,000 BTUH), EDR on water 3300 (495,000 BTUH).

I looked in the library in the beacon boiler books but
did not see any thing on crane 4-12 series B-4

the b-4 large seems
to not be mentioned in my book..but i have a ''12 series Sunnyday Oil Fired'' which has a boiler #4-12 listed under it..its made by Crane..could that be the one your looking for?

this boiler is in a large 28 unit appartment.
what is the firing rate or water rating of your 4-12?

well that can't be the one then
cause its rated at 83500 btu...there is another boiler listed simply as ''crane #4 sectional oil fired'' and it has a 4-12 listed under it also..its rated at 3810 sq.ft. steam, 6095 sq.ft. water..does that one sound right? i'm looking in the ''hydronic rating handbook''by tom byrley.
