thick slab over subfloor

If anyone has done a thick slab pour over a subfloor coudl you give some advice. Did you use the special mix, did you use a bond breaker, dimendions of control joints, did you force cracks with drywall corners? What about mesh over the tube?

When you say thick slab, and sub floor. How thick is the slab, and is the sub floor designed for this type of loading.
