flat plate heat exchanger problems

Has anyone had problems with the CH series flat plate heat exchangers? We had heard of some rupturing and flooding the refrigerant circuit with water and just experienced our first one this week on a direct exchange heat pump.

Obviosly this is a huge problem on a system that uses refrigerant. Any feedback would be appreciated.
