I took one home with me and filled in all information and left jobsite address and date blank keep plenty of copies on hand. Some towns don't enforce, some towns want one per year, some towns like Revere want one for every job and some cities like Boston want one for every job as well as one for plumbing and one for gas work(every job).

all towns are different

A lot of them ask for them every time but some just want it once a year. I usually don't fill it out unless they ask for it. But I think it's supposed to be for every job.
