boiler pressure release valve blows off

It sounds like a heat exchanger leak. An easy way to check is to make sure the valve allowing water into the boiler is off ( as it should be) and note the syetem pressure. Check it in an hour or 2. If the pressure is climbing find out where the water is coming from.
I have a customer who ( we later found out ) has aggressive water, and pinhole leaks developed in the indirect heat exchanger. this could also happen in a coil.
It took their unit 12 hours to raise the pressure, and we replaced the indirect which stopped the overpressure condition, leaks etc.

Peerless boiler pressure valve problem
Basement filled with antifreeze two days ago, exp.tank shot, pressure valve dripping. Svc plumber replaced tank, valve, installed air vent. Next day,same problem..Svc plumber replaces pressure valve, feeder valve and discovers temp. set at 212! This is a 7 yr old HW boiler...four hours later, same problem. Plumber returns, says that main floor line wasn't purged completely. Adds glycol antifreeze. Next morning, same problem..Called emerg. dispatcher who said I should hit the pressure valve with a hammer! Didn't do it..Again waiting for plumber..HELP!0 -

I've seen where a leak in the domestic coil or indirect heater will allow the boiler to be pressurized beyond 30# making the relief valve leak. But not enough to make it blow off.0 -
