Thermal Solutions Boilers w/ Hot Spots

Have a system w/ 5 Thermal Solution modulating boilers. Last year developed a hot tube on boiler #3. Pulled boiler sent back to Factory. They state water problem. Water/Chemical guy says system ppm and ph are right on. Got a call yesterday from maintenance manager and said #4 is getting hot to the touch on the outside jacket (you can only hold your hand on it for 5 sec.) Told him to shut it off until I could get there. New boilers old piping, strainers are pristine. Circ. pumps have triple duties and all seem to be set properly. Has anyone ever had anything like this happen?? Advise course of action...
