    Fuel or Pressure

Looking at the damage (or lack of) it seems the culprit might have been fuel rather than pressure related. It'll be interesting to follow this one as boiler explosions are rare these days. Again....thank God no one was hurt.

    I would concur with that..

The shell seems to be intact. Whatever launched the CB out the door, if internal, went out the dry back. Would love to see a photo of that backside.

By code any hospital must have full capacity if any one boiler is out of commission, but this seemed to take out some breeching (ya think?!) and probably took out the steam header. My bet would be, being a hospital this was high or medium pressure steam for autoclaves and laundry. They said it took out the AC also, so this may have been serving some absorbers... As said, thank God no one was hurt...

Tough to tell... Looks like a 4 pass wet back from what I can see. Mabe I just can't see it well but the breaching looks to be coming out the front of the boiler. Unlikely that a furnace side explosion would have caused the rear to give out if so. My best guess is actually a steam side. I could be wrong.

Most Cleaver Crooks steam side explosions happen where the morrisson tube connects to the rear tube sheet. Usually someone gets lazy and doesn't get all of the scale loose during the annual. They are pretty small explosions by steam side standards. Generally the morrisson tube collapses which is why you don't see any shell damage.

There would have to be some serious negligence for it to be a furnace explosion. I could see it mabe if it was operator error but nobody was present. Not sure would love to see more photos.

I would defer to your more hands-on opinion Josh

But aren't CB's dry-backs? They are four-pass. Every hospital I have worked on that had them always had one open, back-end for refractory replacement.

Interesting take on the Morrison tube. That much punch? Wow. (I guess it is like a cannon though...)

And if over 15 psig and no operator present? That is law in MA (over 15 PSIG and 9 BHP). I thought NJ might be similar but will stand by continuing to learn.
