the wrong, or perhaps missing, orifice in either the air or gas feeds.
Blockage midway through the HX is another possibility from broken ignitors accumulating.
Last, but not least, ours had a leaking gasket just prior to getting the heave-ho.
Yank the tank!

HIGH CO problem
My service manager went out on a trouble call on a GC boiler, and could not get the CO down. It kept locking up his analyzer it was so high. He pulled the air filter, which was dirty, but made no difference. In looking at the manufacturers specs, they don't even mention CO. Is it the nature of the beast? I wouldn't think they are allowed to generate more than 400 PPM, even new.
Experience any one? My gut say yank the old and put in a new one...
