Fulton Pulse Boiler

I have a Fulton Pulse Boiler that has had the gas orifice fall out twice. It is a 2" brass plug with holes drilled in it located inside the combustion chamber. Does anyone know what may be causing this and how toi stop it. Combustion analysis is good and boiler is reliable for about a month after reinstalling the orifice.
