Vent Damper Motor Jamming

I have a Fieldcontrols flu vent damper that was installed three years ago along with my then-new boiler. Suddenly, it is jamming and preventing the boiler from starting. If I shut the power to the boiler off, and then on, it will work again. Just wondering if these commonly fail like this and if there is anything I can do other than replace the entire motor module?

Here is the specific part I am referring to - <a href="http://www.pexsupply.com/Field-Controls-46491600-Vent-Damper-Motor-Assembly-Plastic-Base">http://www.pexsupply.com/Field-Controls-46491600-Vent-Damper-Motor-Assembly-Plastic-Base</a>

    Make sure it is

positioned correctly on the flue pipe and not binding due to a poor fit. It is also from my many experiences with vent dampers better to relocate the damper as close to the chimney or vertical vent as possible as the high temps closer to the equipment affect its operation.

    end switch

The end switch is more than likely going bad. Usually takes 10 years.
